With the exception of the handwritten signature fields, all of the fields
on the PDF form are fillable (e.g., text boxes and drop-down choice
menus).  For the signature, software that can annotate PDFs is likely to
be helpful.  If you are looking for free software to complete the form,
I would suggest Firefox as your best bet.  Recent versions of Firefox
on Windows and Linux (and maybe macOS?) can do this as well as recent
versions of Google Chrome on Chrome OS (but not on Windows or Linux).

Another option for annotating PDF documents is the web-based Kami app,
which can be found at:
    https://www.kamiapp.com
A basic Kami web account is free.

Windows
-------

Firefox 125.0.3 on Windows
- supports filling and saving PDF forms
- has built-in pen annotator that can be used for signature on form
  (with either mouse or touchscreen)

Chrome 124.0.6367.202 on Windows
- supports filling and saving PDF forms
- does NOT have built-in PDF annotation capabilities (although there may
  be plugins for this)

Edge 124.0.2478.97 on Windows
- do not have built-in annotation abilities (e.g., no pen annotator)
- does NOT have built-in PDF annotation capabilities (although there may
  be plugins for this)

Linux
-----

- has built-in pen annotator that can be used for signature on form
  (with either mouse or touchscreen)

I am sorry but I do not have access to any machines running macOS.
